Sage Creek Senior Living assists and supervises all residents with all daily activities 24 hours a day.  Our Administrator will work with you upon admission to  develop a specific individual care plan.  The plan will be continuously monitored by our Administrator and team to ensure the plan changes as your needs change.

Sage Creek Senior Living charges a standard monthly rate that includes the following amenities:

• 24 Hour Staff

• Supervision of Medication

• Monitoring of Vital Signs

• Monitoring of Diabetic Testing

• Assistance with ALL Activities of Daily Living

• Grooming/Bathing

• Basic Self Care Training

• Stimulating Social and Recreational Activities

• Appetizing "Home Cooked Meals" including special diets

• Pharmacy Delivery 

• Religious Services/Visits
